How To Join Us
Tell the receptionist you wish to join the surgery.
You will be asked where you live to see if you are within our designated
area and directed accordingly.
You must bring in some form of identification. The following are
acceptable
but if not, please speak to the receptionist:
Your old NHS medical card
Your passport or driver's licence
Any other form of identification
What Will Happen Next?
You will initially have to complete a registration form
followed by a 'new patient healthcheck' with our nurse.
After this, you will be registered for all our services
including being able to see a doctor.
